What steps do I take to get elm for AF. I have most reforged to i119, but for some reason I don't have the helm.

You would have to reset your af memory from the guy in UJ.

does this mean I have to do all af quests over? even the 1st one for the beat cesti? Or can I just go to garlaige citadel and get the helm from the coffer?

You reset the entire quest line, aka beat cesti and on, until you're at the stage when you can open the coffer. Check the porter moogle first, if you haven't already.
